Highlighting a Collaborative Approach to Value-Based Care

This research focuses on developing a value-based care model for managing chronic conditions, using psoriatic arthritis (PsA) as a case study. It examines how payers, providers, and pharmaceutical representatives collaborated to improve care coordination, address challenges in current care delivery, and ensure a holistic approach to patient management.

The project documents the collaborative efforts of these stakeholders, highlighting their shared insights and strategies to tackle issues such as delayed diagnosis, suboptimal patient management, and limited care coordination. The findings outline a framework for enhancing patient outcomes while fostering effective collaboration in healthcare delivery.
